Course Request with PEIMS# and W/O Form Request

Follow these steps to submit the Course Request with PEIMS# and the New Innovation Course Request W/O PEIMS# Form Request


IMPORTANT DEADLINE NOTICE: Forms will not be accepted after the set deadline. Any requests sent after the deadline must be submitted for the next school year. Please review the Course Request Procedures and Timelines document linked below.

Please review the Course Request Procedures and Timelines document before proceeding.

To submit the form, click on Formspace from the Application Home screen. Then click Submit New Form at the bottom left.

The wizard will open and will walk you through the process. On this screen, select General Forms from the Form Category options.

Select the Form Document from the Form Document list, and click on the specific course request form you need (Course Request with PEIMS# or New Innovation Course Request W/O PEIMS#). Please note that available forms depend on your location.

Select the workflow that corresponds to the requesting department.

Example: If submitting a request for a standard Mathematics course at a HS, select the HS Mathematics Workflow. If the course is an AP or Dual Credit, select the HS AP/Dual Mathematics Workflow.

WARNING: If you select the wrong workflow, the form will be denied, and a new form must be submitted.

Fill out all required fields on the form, then click Submit Form.

Tracking Your Request Status

Once submitted, the approval workflow starts automatically. To track progress:

  1. Navigate to My Forms and click on your form title.

  2. Look at the Workflow panel in the upper-right corner:

    • Under the History heading, you will see completed workflow steps with the date and time stamp of completion.

    • Under the Remaining heading, pending workflow steps will appear.
