Course Request with PEIMS# and W/O Form Request
Follow these steps to submit the Course Request with PEIMS# and the New Innovation Course Request W/O PEIMS# Form Request
Forms will not be accepted after the set deadline. Any requests sent after the deadline must be submitted for the next school year. Please review the Course Request Procedures and Timelines document linked below.
To submit the form, click on Formspace from the Application Home screen. Then click Submit New Form at the bottom left.
The wizard will open that will walk you through the process. On this screen select General Forms from the Form Category options.
You will then need to choose a form from the Form Document list. Simply click on the desired form (depending of your location you might have more forms than what is displayed below).
Select the appropriate Workflow based on what department you are requesting the course for, and the form will appear for you to begin filling it out.
Example: If you will be submitting a course request for a Mathematics course then select the Mathematics Workflow. If the course is an AP or Dual credit course for Mathematics then select the AP/Dual Mathematics Workflow.
If you select the wrong workflow then the form will be denied and a new form will need to be submitted.
Once you have filled out all required fields on the form click on Submit Form to submit the form.
The workflow stars once the form is submitted. You can track the workflow by select the form title under My Forms. Look under Workflow in the upper-right corner.
Under the History heading you will see completed workflow steps with the date and time stamp of completion.
Under the Remaining heading, pending workflow steps will appear.
